If your University has a formal exchange agreement with State College of Applied Sciences in Skierniewice and you have been selected to study as an Erasmus student, you must complete the following forms:
You should not forget to send the Transcript of Records (translated into English) from your home institution which gives information on the subjects you have attended at your university before the start of your Erasmus student mobility.
REMEMBER, AFTER YOU ARE ACCEPTED, BUT BEFORE YOU COME TO STUDY AT OUR SCHOOL,
in order to promote and guarantee the safety and protection of the participants in their project,
we need you to send us documents proving that you are insured against the risks linked to your participation in these activities you have been accepted for. The Erasmus+ Programme does not define a unique format of insurance, nor does it recommend specific insurance companies. Yet, your insurance must cover the following areas:
- wherever relevant, travel insurance (including damage or loss of luggage);
- third party liability (including, wherever appropriate, professional indemnity or insurance for responsibility);
- accident and serious illness (including permanent or temporary incapacity);
- death (including repatriation in case of projects carried out abroad);
- If applicable, it is strongly recommended that participants are in possession of a European Health Insurance Card.
This is a free card that gives access to medically necessary, state-provided healthcare during a temporary stay in any of the 28 EU countries, Iceland, Liechtenstein, Norway and Switzerland, under the same conditions and at the same cost (free in some countries) as people insured in that country.
More information on the card and on how to obtain it is available at http://ec.europa.eu
All application documents, duly signed and stamped by your home institution, must be sent by post directly to:
